ACTS - Acts, Compassion, Teamwork, and Support - is a special way to assist Concentra colleagues facing unexpected hardships beyond their control, which result in a financial hardship, where all other means of financial assistance have been exhausted. During these severe situations, Concentra wants colleagues to feel supported and know they belong to a caring colleague community. The ACTS program is funded 100% from colleague donations. All colleagues are able to contribute as little as $1 per pay period and/or make a lump-sum contribution to the fund.

All regular, full-time and part-time colleagues who are in good-standing are eligible to request assistance.

Examples of qualifying needs include, but are not limited to:

• Disaster assistance (e.g., floods, hurricanes, tornados, earthquakes, fires, etc.).
• Medical expense assistance (catastrophic, medical situations).
• Family emergencies (extreme financial hardship).

Requesting ACTS Assistance

Two ways to request assistance from the ACTS fund are:

1. You may request assistance by faxing or mailing a completed ACTS Request Form along with supporting documentation.
2. You may nominate a fellow colleague who you believe has suffered a catastrophic event that has caused a financial hardship.

An ACTS Request Form must provide the following information:

• A complete description of the catastrophic event that led to the financial hardship.
• The specific need and associated dollar amounts.
• Other financial resources which may be available.
• Supporting documentation to substantiate the requested amount.
• A signature by the person requesting the assistance.
