TUITION REIMBURSEMENT (EDUCATIONAL ASSISTANCE)

At Concentra, we support continuing education and strongly encourage all colleagues to expand their job skills and industry knowledge.  Concentra may financially assist further education and training in fields related to current position.

Eligibility

  •  Full-time colleagues (minimum 30 hours per week) who have completed one year of service may receive reimbursement for certain tuition expenses, up to an annual amount of $1,500 maximum. 
  •  Reimbursement may be conditional upon successful completion of the course, attainment of a minimum mark or grade (must receive a "B" or better), and remain with Concentra for a one-year time period from the date of reimbursement.
  • Prior management approval is required and is contingent upon the availability of educational funds. 
  •  Educational activities should not interfere with your work, your schedule or your performance.

Concentra may reimburse a colleague for certain necessary work-related certifications and license renewals, as necessary.  As with any reimbursement, management approval is necessary. 

Colleagues are expected to schedule class attendance and to complete study assignments outside of their regular working hours.  

Enrollment Procedures

1.  Complete a Tuition Reimbursement form (see Forms) and forward to manager/supervisor for approval prior to the start of course.

2.  Attach course description, nature and purpose of course of study, and cost of course.

3.  Mail completed application to Corporate HR Benefits Department, 5080 Spectrum Drive, Suite 1200W, Addison, TX  75001.

Reimbursement Procedures

1.  Upon completion of course, send a copy of the tuition receipt and grade report to Corporate HR Benefits Department. 

2.  Request for reimbursement must be submitted no later than 30 days after completion of the course or receipt of grades.

3.  Reimbursement checks are made payable to colleague.

4.  Colleagues who are terminated during enrollment due to reduction in force or job elimination, or who are unable to complete an approved course because of the Company, will be reimbursed for the full amount of costs incurred up to the date of termination or transfer. 

5.  Colleagues who voluntarily leave the Company or who are terminated for misconduct will not be reimbursed.
