BASIC LIFE and BASIC ACCIDENTAL (AD&D) INSURANCE
Concentra provides Basic Life and Accidental Death and Dismemberment (AD&D) insurance coverage to all eligible colleagues through the Lincoln Financial Group. Colleagues are automatically enrolled in both Plans. You must designate a beneficiary for this benefit even if you do not elect Optional Life Insurance.
COMPANY-PROVIDED BASIC LIFE AND AD&D INSURANCE
Concentra pays the full cost to provide you with Basic Life Insurance at one times your annual base pay (up to $200,000), and an equal amount of AD&D insurance. All amounts are rounded to the next higher $1,000. You must designate a beneficiary for this benefit even if you do not elect Optional Life Insurance.
OPTIONAL ACCIDENTAL DEATH & DISMEMBERMENT (AD&D) INSURANCE
Optional AD&D insurance may be elected as a supplement to Basic AD&D. Family coverage may also be elected to provide coverage for eligible dependents.
Coverage is available in increments of $25,000 (also known as units) up to a maximum of $350,000. Elections for coverage over $250,000 cannot exceed 10 times your annual base pay. Coverage for dependents is a percentage of your elected coverage.

OPTIONAL LIFE INSURANCE
For Colleagues
You may purchase additional life insurance at group rates through Lincoln Financial Group for yourself and your eligible dependents if you enroll in optional colleague life insurance. The amount of coverage elected for spouse/domestic partner and/or child(ren) must be equal to or less than your total life coverage (basic and optional combined).
You must provide Evidence of Insurability (EOI) and receive approval from the insurance company to:

Elect optional life insurance in excess of $500,000 for yourself or coverage of $100,000 for your spouse/domestic partner, or
-
Increase your current coverage more than one times your annual base pay during annual enrollment.
Colleagues who elect coverage in either of these situations during annual enrollment, but do not submit a completed EOI Form to Lincoln Financial Group postmarked by the deadline announced, will be increased only one times annual base pay. Optional Life Insurance may require approval from the insurance carrier and may have a delayed effective date of coverage.
To purchase Life coverage on your spouse/domestic partner and/or children, you must purchase additional amounts of Life Insurance above the Basic Life provided by Concentra.

OPTIONAL GROUP VARIABLE UNIVERSAL LIFE (GVUL)
For Providers/Clinicians and Officers
You may purchase additional life insurance at group rates through MetLife for yourself and your eligible dependents if you enroll in optional GVUL. The amount of coverage elected for spouse/domestic partner and/or child(ren) must be equal to or less than your total life coverage (basic and optional combined).
You must provide Evidence of Insurability (EOI) and receive approval from the insurance company to increase your current coverage for yourself or your spouse. Coverage amounts decrease at age 65. Effective date of coverage may be delayed pending approval from insurance provider.
Tax-Advantaged Investment Opportunity
The Plan offers you the opportunity to contribute extra premiums for investment into your certificate. These extra premiums have the potential to grow tax-deferred and build cash value. Extra premiums can be invested in your choice of one or more of 22 investment portfolios. Please refer to your MetLife enrollment packet for more information on the investment options.

To enroll or change your Optional GVUL elections or receive premium quotes, please visit MetLife.com, call 1.800.846.0124 (Monday - Friday, 8am to 5pm CST) or refer to your MetLife enrollment/change packet for complete instructions. Colleagues also have the ability to enroll in GVUL through a web portal, MetLife GVUL website.
