LONG-TERM DISABILITY (LTD)

Basic Long-Term Disability is paid in full by Concentra to provide protection against total loss of income during an extended illness or injury.  After 180 consecutive days of disability, basic LTD will pay 50% of monthly base pay up to a maximum monthly benefit of $5,000.  All eligible colleagues are automatically enrolled in basic LTD.

Voluntary Long-Term Disability Plus supplements the Company-provided basic LTD by increasing the benefits level 10% and reducing the waiting period. After 90 consecutive days of disability, LTD Plus will pay 60% of monthly base pay up to a maximum monthly benefit of $10,000.
