VOLUNTARY SHORT-TERM DISABILITY (STD) COVERAGE

Your financial security could depend on enrolling in Voluntary Short-Term Disability Insurance.  This benefit helps to replace a colleague's pay if unable to work due to a non-work-related accident or illness, including pregnancy.

After being unable to work for thirteen (13) consecutive days due to a disabling illness or injury, STD benefits will begin provided required paperwork has been submitted to Lincoln Financial, the STD carrier.  STD benefits provide 60% of base weekly pay, up to $1,500.00 per week, for a maximum of eleven (11) weeks.  (In certain states, STD benefits will be reduced by state-provided disability payments.)

If enrolling outside of initial new hire enrollment period, a colleague must complete an Evidence of Insurability (EOI) form and mail directly to Lincoln Financial Group at address provided on form.  (EOI form is available under Forms.)  Coverage will be effective only upon notice of approval from Lincoln Financial Group.
