Concentra offers various health plans for medical, dental, and vision coverage.

 

In 2012 our medical plan offers financial incentives for colleagues to improve their health through participation in the Concentra Wellness Program. You can receive up to $1,040 in 2012 toward the cost of your medical premiums if you:

 

  • Participate in an online Wellness Profile (WP) and biometric screening during the Wellness Program testing period,
  •  Complete health coaching sessions as required, and
  • Qualify for Healthy Reward incentives by meeting healthy values.

 

As an extra benefit, our medical plan pays 100% (no co-pays or deductibles) for preventive care received in-network. The plan does not cover out-of-network preventive care.

 

Also new for 2012, when you enroll in the Humana Consumer-Directed Health Plan (CDHP), you are automatically enrolled in HumanaVitality rewards program, which rewards its members for making healthy choices and striving to achieve wellness goals — with rewards like brand name merchandise, hotel stays, and more.
