COBRA

 

The Consolidated Omnibus Budget Reconciliation Act (COBRA) allows colleagues to continue coverage up to 102% of the full cost of the medical, dental or vision plan when coverage ends. A COBRA election must be made within 60 days from the date coverage ends (or of COBRA notification, whichever is later).

 

Notice of your right to continue coverage under COBRA, enrollment forms, contact information, and other details will be mailed to your last known address by Concentra's COBRA administrator, Ceridian. The length of continuation ranges from 18 - 36 months depending upon the qualifying event that resulted in the loss of coverage.

 

 

When Coverage Ends for Medical, Dental And Vision Plans.....
Coverage for you and/or your covered dependents under the medical, dental and vision plans will end the last day of the month in which you or a covered dependent either:

  • Terminates employment at Concentra;
  • Ceases to meet the eligibility requirements due to a change in employment status or reduction in hours to fewer than the minimum number required for plan participation;
  • Ceases to meet the criteria for a dependent child(ren) age 25 or younger;
  • Ceases to meet the eligibility requirements as a dependent (i.e., change in marital status or eligibility for other group plan);
  • Fails to pay the required premium payments during a leave of absence.
