The HumanaDental Traditional Plus Plan provides the freedom to visit any dentist for covered services. The same level of coverage exists whether a dentist is in or out of the network. However, out-of-pocket costs will be lower if you visit a participating dentist due to negotiated discounts through HumanaDental. You will receive a separate HumanaDental ID card if you enroll in the plan.

2011 DENTAL PLANS
MetLife Dental PPO Plan is the Concentra plan providing the freedom to visit any licensed dentist for covered procedures. The same level of coverage exists whether a dentist is in or out of the network. Because of negotiated discounts, out-of-pocket costs will be lower if you visit a network dentist. Also, MetLife network dentists will typically file the claims and charge only the out-of-pocket expenses, if any.

Note: MetLife Dental does not provide insurance cards. If you visit a network provider the dental office will ask you for specific personal information to file the claim.

Aetna Dental DMO Plan Each colleague and covered dependent, on this plan, chooses a primary care dentist from Aetna's DMO network. A network provider must perform all services and referrals, and pre-authorizations are required for specialty care. Colleagues pay a pre-determined co-pay amount based on a fixed schedule for dental services.
